Two young British doctors, Chris and Xand van Tulleken, who also happen to be identical twins, immerse themselves in radically different cultures in four of the most extreme places on earth.

In this first episode they travel to Equatorial Africa to live for a month with the Bayaka pygmies who inhabit the densely-forested, swamp-infested Congo Basin, regarded as one of the sickest places on earth. Malaria, ebola and HIV all originated here.

It's a nerve-wracking environment for a pair of well-informed doctors but they want to learn how the pygmies survive and thrive in such a relentless place.

The local medicine man introduces the twins to the panoply of plants and barks used to treat malaria and create a local version of Viagra.

They go hunting for hibernating crocodiles and watch in awe as the men climb thin vines right to the top of the canopy to smoke out bees and gather rich, sweet honey.

But when a baby becomes ill in camp, Xand in particular finds his ethics and modern medicine techniques stretched to their absolute limit.

With characteristic enthusiasm, the twins immerse themselves in the Bayaka's hunter gatherer lifestyle, but quickly realise how genetically unsuited they are to this environment. Being a bit shorter might be more useful, for starters.

They leave in awe of how the swamp forest is the Bayaka's everything - it is their shelter, their food source, a living pharmacy and it shapes their extremely fit bodies.
